About the Role:
Vale of Glamorgan Council are looking for a Social Worker to join the Intake & Assessment Team.
Our function is to: Provide a timely response to Safeguarding concerns, undertake sec 126 enquiries and liaise with partners to develop safe care & support plans Respond to people experiencing crises through assessment, support and family liaison and the commissioning emergency placements/care packages when necessary.
Contracted Hours: 37 hours per week (Full-Time)
Key Responsibilities:
• To undertake assessment of citizens in accordance with statutory duties under relevant legislation.
• To plan and commission services to enable adults with care & support needs and carers with support needs to overcome the barriers to achieving the personal outcomes and monitor and review the care plans through co-production with citizens and carers
• To work in partnership with citizens with care and support needs, carers with support needs and adults at risk to provide the best opportunities for people to live fulfilled lives in a way that matters to them.
• To negotiate and co-ordinate applications for resources and to work in partnership with internal and external partners both statutory and voluntary on behalf of citizens and carers
• To undertake case management responsibility for a specific caseload and prioritise workload demands appropriately.
• To maintain up to date electronic – case files and relevant documents, providing accurate records relating to performance management and other statistical data as required.
• To prepare for and participate in supervision and Personal Development Review (#Its About me) sessions with supervisor/ line manager, and identify personal development and training needs to maintain Registration with Social Care Wales.
• To undertake training to support continuing professional development to ensure that the Council is able to retain the provision of its statutory functions, including a willingness for staff to undertake further training to become Approved Mental Health Practitioners, or other more specific, senior roles.
• To supervise or mentor other staff or students where appropriate and offer input into training programmes and Team Practice Issues meetings where appropriate.
• To take part in office duty rotas and other activities associated with a multi-disciplinary team.
• Ensure compliance with the Council’s Financial Regulations, Standing Orders, Policies and Procedures
• To implement the principles of the Council’s Equal Opportunity Policy whilst carrying out the above duties
• To adhere to Health and Safety legislation / relevant Council policies and procedures and to take reasonable care for the health and safety of yourself and other persons who may be adversely affected by your acts / omissions.
• To implement the principles of the Council’s Environment Policy while carrying out the above duties.
• To report without delay any safeguarding concerns to the appropriate safeguarding officer.
• Such other duties and responsibilities commensurate with the grade and in accordance with the general character of the job as may reasonably be required by the Chief Officer from time to time.
Requirements:
- Qualified Social Worker
- Registered with Social Care Wales
- Over 3 years practicing work experience
- Enhanced DBS
- Eligible to work in the UK
