About the Role:
The post holder will support the Adult Social Care Team in delivering its core purpose effectively and efficiently—ensuring high standards of care, support, and outcomes for service users.
Operating at Grade I, this role will bring advanced expertise to the team, handling the most complex casework and contributing to critical decision-making processes. The post holder will also play a key role in the ongoing development of social work practice, promoting high-quality and consistent approaches across the service.
We operate a hybrid working but staff are expected to be working in the office or out at clients 4 days per week with one day at home for admin.
Key Responsibilities:
- Deliver social work services in line with legislation, policies, and best practices.
- Provide advice, information, and emotional support to individuals and carers.
- Assess needs and develop action plans to manage risk and meet identified outcomes.
- Respond effectively to crisis situations within statutory frameworks.
- Safeguard vulnerable adults by adhering to adult safeguarding policies and legislation.
- Work collaboratively with individuals, families, carers, groups, and professionals to drive positive change.
- Plan, implement, and evaluate care and support plans using a person-centred, asset-based approach.
- Address risk-related behaviours and advocate on behalf of service users.
- Participate in decision-making forums and multi-agency teams.
- Manage your own caseload, keeping accurate, up-to-date records and reports.
- Contribute to resource management and service delivery improvements.
- Apply current research and knowledge to enhance social work practice.
- Maintain professional standards and engage in continuous professional development.
- Use IT systems efficiently to support case management and information sharing.
- Navigate complex ethical issues and maintain accountability in all areas of work.
Requirements:
- Qualified Social Worker
- Must be at least 2 years post qualified with experience in Community Mental Health or Hospital Discharge (Mental Health)
- Registered with Social Work England
- Eligible to work in the UK
