About the Role:
This role within the 16+ Team at Caerphilly County Borough Council is focused on delivering high-quality services to children and families in line with their assessed needs. The postholder will work from an agreed assessment plan to ensure that support is tailored, appropriate, and effectively meets those needs.
All work will be carried out in accordance with statutory responsibilities and the requirements set by the Directorate, ensuring compliance with relevant legislation, policy, and best practice. The aim of the role is to provide responsive, person-centred services that support positive outcomes for young people transitioning into adulthood.
Contracted Hours: 37 hours per week (Full-Time)
 Key Responsibilities:
- To provide high quality assessment and/or care management to children and families who have been assessed as requiring ongoing intervention.
- To review intervention within statutory timescales ensuring plans for children are achieving the identified outcomes.
- Demonstrate professional competence in social work practice.
- To contribute to service development initiatives.
- Ensure effective multidisciplinary working with partner agencies.
- To represent the service at Court and/or Panel meetings.
- Manage and be accountable, with supervision and support, for your own social work practice.
 Requirements:
- Qualified Social Worker
- Registered with Social Care Wales
- Prepared to work flexible hours, within and beyond the boundaries of the authority, which may include evenings and weekends.
- Up to date knowledge of current legislation Research and Practice issues in childcare.
- Eligible to work in the UK
