About the Role:
An exciting opportunity has arisen within the Care Home Review Team. We are seeking a part time qualified Social Worker with knowledge and experience in undertaking assessments, reviews, devising interventions while responding to and managing risk. An understanding of the current pressures experienced in Health and Social Care sector is essential as well as the ability to work under pressure in a fast-paced environment. This is a challenging and rewarding role and will require the successful candidate to work in a multi-disciplinary setting; to consult with service users and their representatives to ensure their needs and rights are promoted while practicing within policy and legislative frameworks. Consideration will only be given to applicants who meet the essential qualifying criteria for this post.
đź’ĽKey Responsibilities:
- Assume care management responsibilities by assessing service users’ needs and risks, delivering direct interventions to individuals and small groups, participating in supervision sessions, and maintaining accurate records in accordance with the council policies and the Data Protection Act.
- Carry out enquiries and make recommendations on whether a person requires compulsory protection measures.
- Investigate, assess, and develop risk management plans, ensuring they are regularly and robustly reviewed.
- Prepare reports and make recommendations to hearings, courts, or tribunals, as required.
- Practice within the relevant legislative framework, ensuring compliance with national and local guidance, policies, and procedures.
- Provide advice and guidance to individuals and families, liaising and advocating on behalf of service users with Council services and external organisations where necessary.
- Promote and raise awareness of the service and examples of good practice among colleagues and wider stakeholders.
- Maintain accountability for continuous professional development, competency, and performance through self-assessment, evaluation, and consultation with a Line Manager, following up on identified development needs.
- Work in line with the Personalisation Agenda, including self-directed care and co-production.
- Adhere to and promote the Code of Practice for Social Service Workers and the Council’s Code of Conduct.
- Undertake ongoing training relevant to the role.
 Requirements:
- Recognised professional social work qualification (at degree level or equivalent)
- Registered with SSSC or required to be registered on commencement of appointment.
- UK Driving Licence preferred
