About the Role:
You will be an integral member of the team, where you will work in partnership with colleagues from Adult Social Work, Adult Community Nursing, Occupational Therapy, Physiotherapy, Public Health Improvement and Patient Flow.
Your primary responsibility will be to work with people in the community to assess their need for care and support and to help formulate and review outcome-based plans. This will be done in conjunction with Carers and families where appropriate, and alongside colleagues in the Home Team. You will also link closely with colleagues in the wider Health and Social Care Partnership and other agencies. Experience of working in a health or social work setting is essential and experience of assessment and care management is desirable. A good working knowledge of Self-Directed Support would be beneficial.
đź’ĽKey Responsibilities:
- Provide advice and guidance to individuals, families, carers, groups, and communities.
- Gather information and undertake assessments, supporting the development of plans, ensuring appropriate risk management and engagement with service users.
- Maintain records of work undertaken with service users in accordance with service requirements.
- Liaise with appropriate agencies on behalf of service users and their families or carers, and advocate where necessary on their behalf.
- Participate in formal supervision as required by the Social Work supervision policy.
- Critically review professional practice and performance in conjunction with line manager.
- Undertake continuous professional development as required within the context of a continuous learning approach.
- Participate in team meetings, case conferences, and multidisciplinary team meetings or other meetings concerned with individual service users or aspects of services as required.
- Participate in the Office Duty provision, including acting as an Appropriate Adult as appropriate.
- Promote public awareness and understanding of our Social Work service and its remit, and encourage community involvement in services where appropriate.
- Demonstrate tasks to students and/or trainees as appropriate.
- Undertake other appropriate duties as required by the Chief Social Work Officer or their nominee.
 Requirements:
- Recognised professional social work qualification (at degree level or equivalent)
- Registered with SSSC or required to be registered on commencement of appointment.
- UK Driving Licence preferred
