About the Role:

Opportunities have arisen for suitably qualified Social Workers within our Hospital Assessment Team.
Our Hospital Assessment Team is a well-established and experienced team which is co-located within an integrated building providing the full range of adult social work and primary health services.
You will be part of a team working across a range of professionals and organisations in acute and community settings. Our focus is on person-centred and timely discharge via a variety of discharge pathways with our first choice being a return home where possible. This role is challenging and fast paced with a number of service developments being implemented.
We are seeking Social Workers to assist in the delivery of our statutory duties and contribute to complex care management and discharge pathways. As such you will be highly motivated with experience of promoting independence and personalisation, be confident in managing risk and looking for creative ways to promote independence seeking to link adults back into the local community.
Applicants should hold a professional social work qualification and will have experience of statutory work within community care social work services. Â Experience within the field of Hospital Discharge is desirable.
You will need to hold a driving licence.

- Be involved in the assessment and management of risk and ensure effective care planning and interventions to minimise risk ensuring the highest level of public protection and community safety
- Contribute to the protection of children , families and adults by adhering to and applying the robust systems and procedures in place and thereby contributing to better outcomes for children, adults and families
- Recognise and respond to the need for early interventions that benefit individuals in need of care and protection

 Requirements:
- Recognised professional social work qualification (at degree level or equivalent)
- Registered with SSSC or required to be registered on commencement of appointment.
- UK Driving Licence preferred
