About the Role:
This role involves delivering a high quality, person-centred, accountable statutory social care service, ensuring that vulnerable adults and their families and careers;
- are able to assess, manage and review their own needs and risks; maximise their own independence and self-direct the responses to deliver agreed outcomes
- can access personalised care and support to meet their assessed needs and maintain themselves in the community
- are protected and safeguarded
 Key Responsibilities:
- Provide timely, outcome-focused responses or referrals in line with legal and policy frameworks.
- Respond promptly to crises and urgent needs.
- Support citizen self-assessments and contribute to multi-agency assessments.
- Coordinate care for complex cases, applying specialist knowledge.
- Ensure culturally competent, anti-discriminatory practices.
- Address concerns and complaints through service improvements.
- Identify and address safeguarding, mental capacity, and health care concerns.
- Balance risk-taking with protection in collaboration with partner agencies.
- Assist citizens and carers in assessing risks, setting outcomes, and planning support.
- Identify and leverage personal and community resources.
- Facilitate access to services, equipment, and funding (e.g., direct payments).
- Help resolve conflicts and adapt plans as needed.
- Refer unresolved or complex issues to line managers.
- Coordinate complex support arrangements when necessary.
- Manage caseloads and prioritise tasks under supervision.
- Support help desk functions and maintain accurate case records.
- Produce reports and correspondence for varied stakeholders.
- Stay informed of relevant laws, policies, and best practices.
- Maintain strong working relationships with internal departments and external agencies, including service providers.
- Build expertise in services, conditions, and community resources to inform team and policy development.
 Requirements:
- Qualified Social Worker
- Registered with Social Work England
- Experience of partnership working with citizens to identify and meet need with creative and pragmatic solutions
- Ability to undertake direct work with those who access or need the service
- Able to work proactively individually but also as a team member to negotiate and implement effective outcomes
- Eligible to work in the UK
