About the Role:
We are seeking a Social Worker to join as a Adults Social Care Assessor at Liverpool City Council.
Contracted Hours – 37 hours per week (Full-Time)
Key Responsibilities:

Carry out strengths-based assessments with adults, their families, and carers.

Gather information on health, housing, mobility, finances, and support needs.

Identify eligibility for services under the Care Act 2014.

Work with individuals to develop personalised care and support plans.

Identify outcomes the person wants to achieve and how they can be supported.
-
Promote independence, wellbeing, and least restrictive options.
-
Carry out planned and ad-hoc reviews of care packages and support plans.
-
Ensure services remain appropriate and cost-effective.
-
Involve service users, carers, and providers in the review process.
-
Provide clear, accessible information on local services, community resources, and funding options.
-
Signpost to preventative services, voluntary organisations, and health services.
-
Encourage use of self-help and informal support networks.
-
Determine if the person meets national eligibility criteria under the Care Act.
-
Escalate complex or borderline cases to qualified social workers or senior staff for further review.

Identify potential risks such as self-neglect, falls, isolation, or abuse.
