About the Role:
We are seeking an experienced Social Worker who is committed to working in Mental Health, in a role with a key focus on working with people working in placements and supporting them to move into a more independent setting. The Adult Social Care Mental Health Service is part of a cutting edge alliance, namely the Living Well Network Alliance (LWNA) providing services through a model of integrated delivery with key alliance partners: South London and Maudsley NHS Foundation Trust, Certitude, Thames Reach and Lambeth NHS Clinical Commissioning Group. As a Social Worker you will deliver a quality, responsive, safe and person-centred service to support and promote the well-being of Lambeth residents. 35 hours per week
Key Responsibilities:
- To undertake joint assessments on vulnerable users referred to the team, and to protect users and others in emergency situations. Taking into account the wishes of individuals and their carers.
- To participate in risk assessments in relation to clients and others.
- To manage a caseload and act as Care Co-ordinator for users on the CPA.
- To jointly plan relevant responses to meet individual needs in consultation with all other interested parties.
- To establish links with Children and Families, for the purpose of making referrals and attending case conferences regarding children who are potentially at risk.
- To secure the delivery, within the financial resources available, of services to meet assessed needs.
- To maintain an effective system for co-ordinating, monitoring and reviewing care plans meeting service standards; to identify gaps in services and to advise the line manager accordingly.
- To maintain a range of costed resource information accessible to users, carers and other colleagues.
- To maintain up-to-date case records in line with department standards.
- Willingness to work flexibly according to service requirements.

- Qualified Social Worker
- Registered with Social Work England
- Eligible to work in the UK
