About the Role:
We are seeking a dedicated and experienced Mental Health Social Worker to join our Integrated Mental Health and Wellness Team. The successful candidate will be responsible for delivering high-quality social work services, with a focus on Care Act Assessments, care reviews, safeguarding, and care coordination under the Care Programme Approach (CPA) framework.
 Key Responsibilities:
- Conduct comprehensive Care Act Assessments and review care plans in line with legal and clinical guidelines.
- Coordinate care for service users under the CPA Framework, ensuring that support is person-centred and recovery-focused.
- Undertake safeguarding enquiries and contribute to multi-agency safeguarding processes.
- Manage a caseload of approximately 25 clients, maintaining accurate and up-to-date records in line with statutory requirements.
- Participate in a Duty Rota, which includes screening, triaging, and responding to new referrals to the service.
- Work collaboratively as part of a multidisciplinary, integrated mental health team, promoting holistic support and continuity of care.
- Engage in regular supervision and contribute to service development through reflective practice and team meetings.
 Requirements:
- Qualified Social Worker
- Registered with Social Work England
- Experience in adult mental health services, including work under the Care Act and CPA
- Enhanced DBS
- Eligible to work in the UK
