About the Role:
Salford City Council are wishing to recruit two experienced social workers to act as Mental Health Commissioning and Review Officers within Salford’s community mental health services. The postholders will be based within Prescott House Community Mental Health Team (CMHTs) situated in the Little Hulton area and Ramsgate House situated in the Lower Broughton area. The primary focus will be upon completing 12 month statutory reviews of mental health care packages in accordance with Care Act and s.117 Mental Health Act.
 Key Responsibilities:
- To undertake allocated reviews of support packages and placements that fall within the funding remit of the GMMH Commissioning Panel where there is no allocated Care Coordinator.
- To ensure care and support plans for packages of care are person centred, strengths based, recovery and outcome focussed, meet assessed needs and demonstrate best value for money.
- To implement required changes to care packages subject to review/re-assessment including commissioning enhancements to care packages or new placements as required.
- To undertake new assessment and commissioning work for individuals under the CMHT who are not subject to care coordination as directed by the team manager where there is workload capacity for this.
- To undertake assessments in accordance with the Care Act (2014), s.117 MHA aftercare and the Mental Capacity Act (2005) as required.
- To undertake Court of Protection work and provide witness statements where required.
- To complete associated casework where this is required to ensure assessed needs and outcomes are met (e.g. applications for appointeeship etc).
- To support and advise Care Coordinators in relation to the commissioning and review processes.
- To contribute to the oversight of social care commissioning and review work in the CMHT.
- To support effective patient flow by prioritising commissioning and review work for inpatients within acute care.
- To participate in and support the work of the GMMH Commissioning Panel.
- To contribute to the oversight of Residential and Nursing placements, to promote positive movement through the service is facilitated wherever appropriate.
- To participate in multi-disciplinary team meetings; any other relevant forums and meetings.
- To provide training to staff in relation to commissioning and review systems and processes.
- To contribute to maintaining and updating appropriate guidance in relation to relevant processes and procedures.
- To ensure that recording systems accurately reflect activity on commissioning and reviews, and to contribute to monthly monitoring reports as requested. This will relate to PARIS, Liquid Logic and GMMH Commissioning panel records.
- To prepare reports and undertake audits for specific purposes as required.
- To ensure that agreed actions and outcomes are fully evidenced, to provide a robust audit trail of activity.
- To contribute to updating and maintaining the GMMH schedule of reviews and related data.
- To contribute to the implementation of personalisation and promotion of Direct payments across mental health services.
- Develop and maintain effective working relationships with Salford City Council, Adult Social Care at Salford Care Organisation, other partner agencies, accommodation providers and the voluntary sector to ensure improved Service User pathways and service provision.
- To alert relevant agencies in areas where concerns are raised, for example making referrals in relation to child and adult safeguarding and alerting the Care Quality Commission.
- To be willing to undertake AMHP training or if qualified to participate in the AMHP rota.
 Requirements:
- Qualified Social Worker
- Substantial prior experience working with individuals having mental health conditions
- Registered with Social Work England
- Eligible to work in the UK
