About the Role:
Warrington Borough Council are looking for a qualified Social Worker to join their dedicated Hospital Discharge Team.
This post is predominantly based on-site with some hybrid working available.
Key Responsibilities:
- Carry out duties with full regard to the relevant legislative framework, the Council’s Corporate Plan, and other relevant policies such as the Corporate Equality and Diversity Policy, Health and Safety Policy, and Social Inclusion Strategy.
- Complete assessments under the legislative framework, including identifying risk and the need for protection, and determine the appropriate intervention.
- Liaise and negotiate with customers, carers, hospital colleagues, and all relevant agencies to ensure services provide the best standards of support and value for money.
- Manage an allocated workload in line with Directorate priorities and Council policies.
- Maintain appropriate and timely electronic records of work undertaken and carry out required administrative procedures.
- Prepare for and attend Time to Talk sessions and staff meetings, and make use of all relevant training and development opportunities.
- Maintain evidence of practice to meet continual professional development requirements.
- Contribute to the team’s duty rota, screen enquiries and referrals made to the service in line with policies and procedures.
- Contribute to the evaluation and development of services and new initiatives by sharing knowledge of theory, skills, and practice with team members, professional groups, and other interested bodies.
- Carry out all duties with due regard to confidentiality and data protection regulations.
- Safeguard and promote the welfare of vulnerable adults and children as part of core responsibilities.
- Undertake such additional duties as are reasonably commensurate with the level of the post.
Requirements:
- Qualified Social Worker
- Experience in Hospital Discharge work
- Minimum of 2 years experience in statutory Social Work
- Registered with Social Work England
- Eligible to work in the UK
