About the Role:
We are seeking a Social Worker to join our dedicated Area Team at Worcestershire County Council.
Contracted Hours – 37 hours per week (Full-Time)
Key Responsibilities:
- Undertake statutory assessments under the Care Act 2014.
- Assess adults’ needs, strengths, and risks, considering their personal outcomes and preferences.
- Develop and review person-centred care and support plans.
- Promote independence, choice, and well-being in all planning.
- Identify and respond to concerns of abuse, neglect, or exploitation.
- Initiate and lead Section 42 safeguarding enquiries.
- Work within multi-agency safeguarding procedures and ensure legal compliance.
- Promote safe outcomes and reduce risk through robust planning.
- Assess mental capacity under the Mental Capacity Act 2005.
- Make or support best interests decisions where capacity is lacking.
- Involve Independent Mental Capacity Advocates (IMCAs) or other relevant advocates where required.
- Respond to urgent referrals, including hospital discharges, breakdowns in care, or immediate safeguarding concerns.
- Arrange emergency services, placements, or support to ensure safety and stability.
- Collaborate with professionals from health, housing, voluntary sector, and mental health services.
- Attend and contribute to Discharge Planning, MDT meetings, and reviews.
- Ensure coordinated and integrated support for service users.
- Assess the needs of unpaid carers.
- Provide support planning for carers in line with the Care Act.
- Signpost or refer to carers’ services and respite support as appropriate.
- Fulfil responsibilities under relevant legislation (e.g. Care Act, Mental Capacity Act, Human Rights Act).
- Prepare case notes, reports, and court statements when needed.
- Adhere to confidentiality, data protection, and local authority policies.
- Support access to Direct Payments, Personal Budgets, and community-based services.
- Work in a strengths-based and outcome-focused way.
- Encourage self-advocacy, peer support, and social inclusion.
- Monitor ongoing support arrangements and conduct timely reviews.
- Adjust care plans as circumstances or needs change.
- Evaluate outcomes and service effectiveness.
- Participate in regular supervision, team meetings, and CPD.
- Stay updated on policy, legislation, and evidence-based practice.
- Contribute to service improvement through feedback and reflective practice.
Requirements:
- Qualified Social Worker
- Registered with Social Work England
- Eligible to work in the UK
